Please print the last page using the "Send to Printer" button located in the upper right hand corner of the page and include it with your payment.
Include with your payment a copy of the LHSAA Medical History form.
If this is the first year to register with the JETS, a copy of the players birth certificate must be sent as well.
Payment and all paperwork must be submitted prior to the first practice.

Welcome to JETS Football. As you know it is our goal to teach young men the game of football and the fun and enjoyment it can bring.
We expect the best from our coaches and players, regarding attitude, discipline, and commitment. There are very few sports that require the time and practice that football requires. We ask that all parents respect the coaches’ time and commitment. The board asks that all parents remain off the practice field; please respect this request.
As you know there is a lot of work that goes on behind the scenes to run a football program. There are many areas that we need volunteers to help. If you are interested in helping, please contact your coach and let him know.
All Players must have a completed LHSAA Medical History form prior to the first practice. You can download this form below.
